November 10, 2010   URGENT SAFETY NOTICE

GUARDIAN FULL FACE MASK POSSIBLE EXHAUST DIAPHRAGM PROBLEM.

 

It has been brought to our attention that under certain conditions and/or
usages, the Guardian Full Face Mask (GFFM) Exhaust Assembly may come loose. If
this assembly comes loose and separates from the housing, you will not be able
to breathe the 2nd stage regulator. This is a result of a defective part with
bad threads. It is extremely important that you conduct a simple test to ensure
your 2nd stage regulator is securely fastened If you find any problem whatsoever
with the 2nd stage, "DO NOT DIVE THE MASK".

URGENT RECALL: OTS Guardian Full face mask at 2009/11/17

Manufacturer Ocean Technology Systems has announced a recall of defective Guardian full-face masks. Due to defective visor clamps, the clear plastic visor may dislodge causing the mask to flood, which poses a drowning hazard to the user.

Ocean Technology Systems has received three reports of visors dislodging. No injuries have been reported.

The recall affects the following Ocean Technology Systems equipment (about 900 units):

Guardian Full-Face Mask (sold from March 2009 through August 2009 for $800)
Owners of Ocean Technology Systems Guardian Full-Face Masks affected by the recall should stop using the defective equipment IMMEDIATELY and contact Ocean Technology Systems to receive a free repair.

Manufacturer: Undersea Systems International Inc., dba Ocean Technology Systems, of Santa Ana, Calif.

Hazard: If significant pressure is applied vertically to the top and bottom of the visor clamp, the clear plastic visor may dislodge causing the mask to flood.

Incidents/Injuries: Ocean Technology Systems has received three reports of visors dislodging. No injuries have been reported.

De_script_ion: This recall involves the Guardian full-face mask which is a scuba diving mask that incorporates the second stage regulator into the mask allowing it to cover the diver's full face.

Sold by: Direct sales and diving equipment retailers nationwide from March 2009 through August 2009 for $800.

Manufacturer: Brownie's Third Lung, of Fort Lauderdale, Fla.


Hazard: The air compressor's components can fail, which could cause a stoppage of air flow. This poses a risk of drowning to users.  Incidents/Injuries: None reported.  
Description: The recalled air compressors are used to provide air to divers. The recalled products include the following models:

SI Tech Recalls Diving Suit Hoses Due to Drowning Hazard; One Death Reported

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Diving Air Hose for Dry Suits

Units: About 65,000

Manufacturer: SI Tech AB, of Brastad, Sweden

Hazard: The hose contains an insert that can dislodge during diving and restrict air flow to the diver, posing a drowning hazard.

Incidents/Injuries: SI Tech has received six reports of hose inserts dislodging, including one that was involved in the death of a diver in Los Angeles, Calif.

Description: This recall involves a dry suit inflation hose that connects a diver’s dry suit to the air supply and allows for the pumping of air into the suit to set up a positive pressure arrangement to help keep it watertight. The hose contains an air flow restricting insert that may be either black, blue or green in color. The batch code is stamped on the threaded metal end of the hose. Remedy: Consumers should immediately stop using diving equipment that contains the recalled low pressure inflation hoses and contact SI Tech for the location of an authorized dealer for a free repair which involves removal of the hose insert, or to receive instructions on how to repair the hose.

Halcyon announces diving equipment recall

2009

 

Halcyon Manufacturing is recalling about 20,300 diving equipment items, because the over pressure valves could fail, allowing the buoyancy compensator devices and the diver lift inflatable devices to leak, posing a drowning hazard.

 

No incidents or injuries have been reported.

 

The equipment was sold at diving equipment retailers and distributors from January 2006 through December 2008 for between $350 and $450 for the buoyancy compensator devices, and between $50 and $275 for the inflatable devices.

 

Consumers should return the equipment to a Halcyon dealer for a free inspection and, if necessary, free replacement of the overpressure valve spring.

September 17, 2008   Aqua Lung America Recalls Apeks Scuba Diving Regulators Due to Drowning Hazard  

 Release #08-402   WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Apeks Second Stage Scuba Regulators  
Units: About 25,000    Distributor: Aqua Lung America of Vista, Calif.   Manufactured in: England

Manufacturer: Apeks Marine Equipment Ltd., of Blackburn, England 
Hazard: These regulators can be missing the diaphragm cover which can cause the diaphragm to become displaced during a dive, allowing water to enter the scuba regulator. This poses a drowning hazard to divers.
Accidents/Injuries: None.  

Description: This recall involves Apeks TX, ATX, and XTX second stage regulators that have never been serviced. Apeks and TX, ATX, or XTX is printed on the regulator. 

Sold at: Authorized Apeks dealers nationwide from February 2000 through June 2008 for between $420 and $1,450.    

Remedy: Consumers should immediately stop diving with these regulators and visit an authorized Apeks dealer for a free inspection and free installation of a diaphragm cover, if the cover is missing.

Release #08-393  


WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. 


Name of Product: Titan DIN 1st Stage Scuba Regulators and Titan/Conshelf DIN Scuba Adaptors


Units: About 6,000  


Manufacturer: Aqua Lung USA, of Vista, Calif.     Manufactured in: France    
Hazard: Over-tightening of the DIN retainer by a technician during installation can result in the retainer breaking under pressure, a rapid escape of air from the scuba cylinder, and the regulator detaching from the scuba cylinder. This poses a drowning hazard to divers. 
Incidents/Injuries: The firm has received one report of a DIN retainer breaking under pressure. No injuries have been reported.  

 

Description: The recalled regulators have a brass DIN retainer manufactured prior to June 2006. The recalled Aqua Lung Titan Din 1st Stage regulators have serial numbers lower than 6062501 stamped on the side of the regulator's body. Recalled Titan/Conshelf DIN adaptors are marked "300 BAR MAX" on the side of the part. DIN adaptors marked "300 BAR MAXI" are not included in this recall. 


Sold at: Authorized Aqua Lung dealers nationwide from January 1997 through September 2008 for about $300 to $450 for the scuba regulators or $70 for DIN adaptors sold separately.  


Remedy: Consumers should immediately stop using the recalled diving regulators and DIN adaptors and return them to any authorized Aqua Lung dealer for a free replacement DIN retainer.

July 2008   EANx32 Recreational Dive Planner 

PADI Americas is recalling a single version of the English Imperial EANx32 Recreational Dive Planner used for calculating dive profiles. A printing error caused a series of numbers to be out of alignment, thus incorrect. The risk of injury from these errors is remote and no injuries have been reported.  


All RDPs included in this recall bear the printing mark "Printed in the USA" located on the back of the table, upper right corner. The product number and production code are located on the lower right corner, also on the back of the table.


The product number and production code affected by this recall are: 
Product No. 60049 Ver 1.2 (Rev 02/03) XXXPDK7  


The product code digits indicated by "X" may vary. The last four digits "PDK7" indicate a recalled table, irrespective of what the first three digits are.  


As a double check, on Table 3, check the 110 foot row under Pressure Group G. The table should indicate a Residual Nitrogen Time (upper number in white) of 13 and an Actual Bottom Time (lower number in green) of 12. If this box is blank, you have a misprinted table. 


We will be notifying consumers certified as PADI Enriched Air Divers after 17 January 2008 via the PADI website, e-mail and other means. They will be directed to stop using the recalled RDP immediately and bring them to any PADI Dive Center/Resort or return them to PADI for a free replacement. Hazard: The swivel, which is attached to a diving mask, could separate
while diving. This will result in a sudden loss of the diver's air
supply, causing the diver to engage in emergency ascent. This poses a
risk of decompression sickness due to rapid ascent or drowning.

Incidents/Injuries: M&J Engineering has received one report of swivels
separating from the masks while diving. No injuries have been reported.

Description: This recall involves all SW-3000 2nd stage 360-degree
swivels for scuba diving masks manufactured between January 2004 and February 2005. The swivel has the date stamped on its body, below the patent number. Swivels that are already repaired have an arrow stamped on the top right corner of the swivel body.

Sold at: Diving retail stores nationwide between January 2004 and
February 2005 for about $60.

Hazard: The swivel, which is attached to a diving regulator, could
separate while diving. This will result in a complete and sudden loss of
the diver's air supply, causing the diver to engage in emergency ascent procedures. This poses a risk of decompression sickness due to rapid ascent, and air embolism or drowning if the diver panics or the emergency ascent procedure fails. Consumers should stop using recalled products immediately unless otherwise instructed.

Incidents/Injuries: The firm has received one report where the swivel
separated during a dive and caused the diver to ascend using the buddy breathing technique. No injury was reported.

Description: The recall involves the HO110 Swivels sold as an
aftermarket regulator component. There are no markings such as a date or production code embossed on the unit. Contact Innovative Scuba Concepts for information on where the recalled units were sold.

Sold at: Specialty retail dive stores nationwide from January 2006
through March 2007 for about $40. They also could have been installed by a dive shop regulator technician.

Manufactured in: Taiwan

Remedy: Consumers should stop using regulators with the swivel attached and contact the dive store where purchased for a refund.

Subject: Pelagic Recalls Digital Dive Computers Due to Decompression Hazard

Name of product: Oceanic and AERIS Digital Dive Computers

Manufacturer: Pelagic Pressure Systems, of San Leandro, Calif.

Hazard: When performing a switch from one gas to another during a dive, the dive computer's display will lock up and not return to the main dive screen that displays dive times. This can cause divers to enter decompression unknowingly or the diver could ascend prematurely, resulting in decompression sickness.

Incidents/Injuries: Pelagic has received a report of two dive computers
malfunctioning. No injuries have been reported.

Description and Models: The recall involves Oceanic-brand ATOM 2.0 dive computers with serial numbers 1 through 2,079 (Revisions 2E, 3A, and 3B) and AERIS-brand EPIC dive computers with serial numbers 1 through 712 (Revision 1A), which can be accessed and viewed on the computer's display. Also, the serial number and date of manufacture are printed on the bottom of the unit (Oceanic ATOM 2.0 from August 23 to November 23, 2006, and AERIS EPIC from October 18 to November 14, 2006). This recall does not include any other Oceanic or AERIS brand dive computers.

Sold at: Authorized Oceanic dealers sold ATOM 2.0 dive computers
nationwide from August 2006 through February 2007. Authorized AERIS dealers sold EPIC dive computers nationwide from October 2006 through February 2007. Both computers sold for between $670 and $950.

Remedy: Consumers should stop using the recalled dive computers and take them to an authorized Oceanic or AERIS dealer to get a free software upgrade.

Manufacturer: Suunto Oy, of Finland

Hazard: These dive computers could incorrectly track dive time, which
could cause incorrect calculation of decompression requirements. This
could lead to decompression sickness.

Incidents/Injuries: None reported

Description: The D9 and D6 diving instruments are advanced,
gas-switching, multi-mode, decompression dive wristop computers. The products included are: the D9 model with serial numbers 62102582 and below; and the D6 model with serial numbers 62103693 and below. The serial number is located on the side of the product. The model number is located on the back of the product.

Sold at: Diving specialty shops nationwide, as well as various Web
sites, from September 2004 through June 2006 for about $1,275 for the D9 (without wireless transmitter) and $900 for the D6.

Manufactured In: Finland

Remedy: Consumers should only use these dive computers with backup instrumentation. Consumers should bring the recalled units to the nearest authorized Suunto dealer for a software update to correct the problem. A free battery replacement and pressure testing will be
provided as part of the free software update service. All updated
products will be marked with an indelible white dot on the back of the
unit, or with an engraved "U" near the serial number.

Name of Product: Scubapro(r) X650 Second Stage Regulator
Units: About 670
Manufacturer: Scubapro(r) with global headquarters located in El Cajon, Calif.

Hazard: A manufacturing error could cause the main housing of some
regulators to change shape over time, which could cause the cover and diaphragm to become dislodged. If this occurs, air flow will be
interrupted and the regulators will no longer function, posing a
drowning hazard to users.

Incidents/Injuries: None reported.

Description and Models: The recall involves some X650 second stage
regulators. All X650 second stage regulators are clearly marked with a
blue capital "X" next to the numbers "650" in white. Affected regulators have a visible bump located on the right side of the unit between the letter "C" and where the mouthpiece attaches to the regulator body.

Sold at: Authorized Scubapro(r) dealers in the U.S. from June 2005
through May 2006 for between $400 and $670

Remedy: Consumers should stop using the product immediately and contact Scubapro(r) to determine if they have an affected second stage regulator. If so, they will receive a free replacement regulator. The company is contacting registered owners of the product directly.

Subject: Oceanic Recalls Digital Dive Computer Due to Decompression Hazard

Date: June 22, 2006; Release #06-194 
Name of Product: Versa Pro 2A Digital Dive Computer
Units: About 6,100
Manufacturer: Oceanic, of San Leandro, Calif.

Hazard: When using the dive computer set for "User Selected Digital
Gauge Mode," the "Elapsed Dive Time" displayed can exceed the actual
elapsed time under water. This can cause divers to ascend before
fulfilling a decompression obligation, resulting in decompression
sickness.

Incidents/Injuries: Oceanic has received one report of the dive
computers malfunctioning. No injuries have been reported.

Description and Models: The recall involves Oceanic-brand Versa Pro 2A dive computers with serial numbers 12000 through 18176 which can be accessed and viewed on the computer's display. The serial number and "r2A" are printed on the side of the unit. This recall does not include any other Oceanic brand dive computer, or Versa Pro 2A while operated in normal decompression dive computer mode.

computers nationwide from March 2003 through April 2006 for between $430 and $640.

Subject: Scubapro® Recalls MK 20 Scuba Regulators for Service

Name of Product: Scubapro® MK 20 First Stage Regulators

Manufacturer: Scubapro® USA, of El Cajon, Calif.
 
Hazard: Over-tightening of the Yoke or Din retainer during annual servicing of these regulators could result in a stress crack and failure of the regulator. If this occurs during a dive, air supply could be interrupted, posing a drowning hazard.

Incidents/Injuries: No injuries or underwater incidents have been reported. Scubapro® USA has received eight reports of units that failed after being serviced.

Description: The recall involves first stage regulators that are marked with the Scubapro brand and identified as the MK20.

Sold at: Authorized Scubapro® dealers nationwide from January 1996 through December 2001 for about $260.

Remedy: Scubapro® USA has been providing a service upgrade kit via the authorized dealer network since May 2005 which contains a Yoke or Din retainer that prohibits over-tightening during annual service. The serial number will appear on the dive computer screen after removing and reinserting the batteries. The Darwin Air is a completely integrated dive computer and pressure gauge with “Dacor Darwin Air” written on the front. It is encased in gray on black heavy-duty plastic. Description: Halcyon is recalling all stainless steel power inflators. They have a stainless steel oral and power inflate button. Inflators made of plastic are not included in this recall. The repaired inflators have a groove at the base of their retainer nut. The BCs sold with these inflators are black and the Halcyon logo is on the front and collar of the BCs.

The date code reads the number of the month followed by a decimal point and the year, for example, "01.95." The dive computers are blue and black with the words "Aladin" and "NitrOx" printed across the front. Aladin Air X NitrOx dive computers with date codes other than "95" are not included in this recall.

The serial number is stamped into the side of the body of the regulator. The regulators have the name "OCEANIC" written on the rubber boot of the first stage. These CDX regulators were sold with the following second stage regulators: Alpha 7, Delta 3, Gamma 2, and Zeta.

The jacket model has the DUI logo printed on the jacket pocket, epaulette, and weight pockets. The wings model has the DUI logo printed on the epaulette and weight pockets. The letters "DUI" are printed on the permanent hang tags found on the Buoyancy Control System. All new OPVs are equipped with yellow pulls.

Description: The recalled regulators have a black body with either a gray, blue, red or yellow faceplate. Printed on the faceplate of the regulator are the words, "Atomic Aquatics"; the model name, "Titanium"; and the model number, T1, T1x, B1 or Z1.

The regulators were manufactured between May 1999 and October 1999, and have serial numbers that range from A07751 to A11276. The serial numbers can be found on a label beneath the rubber mouthpiece.

Description: These recalled Durapro Systems BCDs are the Cirrus, Stratus, Aruba, Slimline and Resort. The low-pressure hoses on the BCDs, part number 302, connects the first stage regulator to the BCD power inflator valve. These hoses are identified with the following markings: "PLIOVIC MULTI PURPOSE 1/4" 1/D (6.4 mm) 250 PSI WP (1.7 MPA)...MADE IN USA...GOODYEAR."

Sold At: Retail dive stores and a mail-order catalogue sold these BCDs from January 1996 through May 1998 for about $179 to $359.

Remedy: Consumers who purchased one of thesea Durapro Systems BCDs should stop using it immediately. The hose from the BCD should be returned to the dive store where purchased for a free replacement hose.

Description: The Guardian dive computer comes in two models, a wrist mount and a hose mount. The computer is about 3 inches wide and 3 inches long. It is black and gray with yellow buttons. "Mares" and "Guardian" are written on the frame of the computer around the screen.

Hazard: The glass in the masks can break into sharp pieces and cut divers. Incidents/Injuries: Kmart has received seven reports of masks' glass breaking, resulting in one diver's hand being cut.

Description: The Healthways Ocean Side dive mask is made of fluorescent yellow, marine blue or raspberry plastic and clear tempered glass. The mask was part of a dive set that included a snorkel and fins. The words "Healthways Ocean Side...Tempered" are printed on the front of the mask. The dive set is packaged in a blue, yellow and white cardboard box labeled in part, "Barbados Healthways Ocean Side...4-Piece Dive Set...Made in China for Kmart." The dive set is visible through a plastic window on the front of the box. The following UPC codes are on the box: 609299960042, 74767136154, 609299960011, 747671361510, 609299960028, 747671361527, 609299960035 or 747671361534.

Sold At: Kmart and Super Kmart stores nationwide sold the dive mask set from February 1997 through August 1997 for about $20.

Remedy: Consumers should stop using these dive masks immediately and return them to the nearest Kmart store for a refund. Consumers will receive $8 for the dive mask or a full refund for the set.

Subject: Scubapro Recalls G500 Second Stage Regulators

Product Name: G500 second stage regulators

Hazard: The VIVA control knobs on these regulators can detach during a dive allowing water to rush into the regulator, which could result in a diver drowning.

Incidents/Injuries: Scubapro has received six reports of the VIVA control knob detaching from the regulator, two of which occurred during a dive. No injuries have been reported.

Description: The G500 second stage regulators involved in this recall have serial numbers less than or equal to 13497450. The serial number is located on the bottom face of the exhaust manifold.

Sold At: These products were sold in diving specialty stores nationwide from March 1997 through June 1997 for around $300.

Remedy: Consumers should immediately stop using G500 second stage regulators involved in this recall and contact their local, authorized Scubapro dealer to arrange for a free replacement of the VIVA control knob. Under no circumstances should consumers dive with these regulators until they have been repaired.

Subject: Light & Motion Industries (LMI) Recalls Underwater Batteries

Hazard: If the battery pack floods during the dive, the battery could overheat later while above water and catch fire.

Incidents/Injuries: LMI has received two reports of Source-12 Battery Packs catching on fire several hours after use. No injuries have been reported.

Description: The battery packs, measuring 7 inches long by 4 inches wide, are black with a clear top. This corrective action involves Source-12 Battery Packs with the following model numbers:

860-0001 through 860-0006

860-0018 through 860-0031

Sold At: Dive shops sold the Source-12 Battery Packs nationwide beginning in January 1990 for about $275. LMI also sold the packs directly to consumers.

Remedy: If consumers notice the battery has flooded during a dive, they should disassemble the battery's retaining ring and bulkhead to prevent a possible fire. Consumers should stop using the Source-12 Battery Packs immediately and call LMI for free repair or upgrade information. Consumers can also return the batteries to the dive shop where purchased and the batteries will be sent to LMI.

Subject: Trident Diving Equipment Recalls Scuba Diving Tank Valve Caps

Product Name: Screw-in type DIN Tank Valve Caps
Units: approximately 500

Hazard: If tank pressure is 3000 psi or greater, the valve cap is not properly inserted, and the air valve is on, the valve caps may fracture and pop out of the air tank, causing injury.

Incidents/Injuries: Trident is aware of one incident of injury where a valve cap allegedly popped off a tank and struck the consumer in the hand.

Description: The valve cap, Trident Model RP73, is intended for use on SCUBA diving compressed air tanks to prevent sand, dirt or dust from contaminating the area around the valve. The black plastic caps measure 1" in diameter and 1" thick. The hexagon-shaped caps can be tethered to tanks by a string attached to a plastic eyelet in the top of the cap.

Sold At: The valve caps were sold between January and June 1995 through Trident's catalog and at dive shops nationwide. The caps cost between $2.00 and $4.00 and were packaged in unlabeled plastic bags.

Subject: DUI Recalls Dry Suit AC Valve

Hazard: The valve may free-flow causing air to fill the dry suit resulting in the diver rising rapidly to the surface or failing to provide sufficient air to maintain buoyancy. The possibility exists of serious injury and/or death, especially if appropriate emergency procedures are not followed.

Incidents’Injuries: The Consumer Product Safety Commission is aware of incidents involving 16 divers which are associated with AC valve malfunction. 

Description: The DUI AC valve is used on SCUBA diving dry suits. The DUI AC valve deflates or inflates the diving dry suit to allow the diver to equalize air pressure while ascending or descending in the water by pressing on the valve. The AC valve differs from the standard 2-valve system commonly used in dry suits, where air intake and outflow are controlled by two separate valves.

Remedy: Owners of dry suits with AC valves attached should immediately stop using the dry suits. DUI asks that all divers return their suits (at DUI's expense) and have the AC valve replaced at no charge. Divers who have one of these AC valves attached to their dry suits should contact DUI on their toll free number 800-327- 8439 to obtain valve replacement instructions and a FREE dry suit check-up. All dry suits returned to DUI will be tested and DUI will fix any leaks in the suit (except for seals) at no additional charge. Further, if the diver wishes to upgrade the valve system, this will be done at the manufacturer's cost of the valves only.
